Brain fog, poor sleep, joint aches, and mood changes can all be part of menopause. Learn the symptoms women often miss and why they matter.

When most people think about menopause, the first symptoms that come to mind are probably hot flashes and night sweats. These are among the most common symptoms of the menopause transition, affecting

           up to 80% of women

            at some point.

But menopause is much more than hot flashes.

The hormonal changes of perimenopause and menopause can affect virtually every system in the body. Some symptoms are subtle, develop gradually, or are easily attributed to stress, aging, or simply having too much on your plate. As a result, many women don’t realize that the changes they are experiencing may be related to the menopause transition.

What exactly is happening during menopause?

Perimenopause is the transition leading up to menopause. During this time, ovarian hormone levels, particularly estrogen, can fluctuate considerably. Menopause itself is defined retrospectively after 12 consecutive months without a menstrual period.

While the experience is different for every woman, the hormonal changes of this transition can affect the brain, sleep, genitourinary system, musculoskeletal system, and cardiovascular and metabolic health.

And importantly, you don’t have to have hot flashes to be experiencing menopause-related symptoms.

           Symptoms beyond hot flashes

           1. Sleep changes

Sleep problems are extremely common during the menopause transition.

Some women have difficulty falling asleep. Others begin waking during the night or find that they wake much earlier than they used to. Night sweats can certainly disrupt sleep, but sleep disturbance can occur even in women who don’t experience obvious hot flashes.

Poor sleep can then create a cascade of other problems, including fatigue, irritability, difficulty concentrating, reduced exercise tolerance, and changes in appetite.

           2. “Brain fog” and memory changes

One of the most frustrating symptoms women describe is a feeling that their brain simply isn’t working the way it used to.

           You may find yourself:

Searching for a familiar word or name

Walking into a room and forgetting why you went there

            Having more difficulty concentrating

            Becoming more easily distracted

            Struggling to multitask

Feeling mentally “foggy” or less sharp

These cognitive complaints are common during the menopause transition.

           The Menopause Society

notes that approximately 40% to 60% of midlife women report cognitive symptoms during this period.

For many women, this can be particularly concerning. It is important to distinguish the common, generally mild cognitive changes associated with the menopause transition from the progressive cognitive decline seen with dementia. Persistent or significant cognitive changes should still be evaluated rather than automatically attributed to menopause.

           3. Mood changes, irritability, and anxiety

Some women notice a change in their emotional baseline during perimenopause.

           You may feel:

            More irritable or short-tempered

            More anxious

            Less resilient to everyday stress

            More easily overwhelmed

Less motivated or more down than usual

Women with a history of depression may be particularly vulnerable during this period, but menopause-related mood symptoms can occur in women without a prior psychiatric history.

And sometimes the symptom isn’t simply “depression.” A woman may tell us, “I just don’t feel like myself anymore.”

           That deserves attention.

           4. Joint and muscle aches

New or worsening aches and stiffness can also occur around menopause.

A woman who has always been active may suddenly notice that her knees, hips, shoulders, or hands are more uncomfortable, or that she needs longer to recover after exercise.

Not every new ache is caused by menopause, and conditions like arthritis, injuries, and thyroid disease still need to be considered. But the timing of new musculoskeletal symptoms can provide an important clue.

           5. Changes in sexual health

Sexual symptoms are common but are often underreported.

Declining estrogen levels can cause changes in the tissues of the vulva and vagina, including:

            Vaginal dryness or reduced lubrication

            Burning, irritation, or discomfort

            Pain with intercourse

            Decreased sexual satisfaction

Some women also experience a decrease in sexual desire.

These symptoms fall within what is called genitourinary syndrome of menopause (GSM). Unlike hot flashes, which often improve with time, GSM can become progressively more bothersome without treatment.

           6. Urinary changes

Changes in estrogen can affect the bladder, urethra, and surrounding tissues.

           Women may notice:

            Increased urinary frequency

            Urgency

            Leakage

            Burning with urination

            Recurrent urinary tract infections

Waking during the night to urinate

Because urinary symptoms have many possible causes, they should not automatically be attributed to menopause. But menopause should be part of the conversation, particularly when symptoms develop during or after the menopause transition.

           7. Headaches and migraines

For some women, hormonal fluctuations during perimenopause can make headaches or migraines more frequent or more severe.

Women who have historically experienced menstrual migraines may notice a change in their pattern as estrogen levels fluctuate.

A new or substantially different headache, however, should always be appropriately evaluated rather than assumed to be hormonal.

           8. Changes in weight and body composition

Many women notice changes in their body composition during midlife, even when their diet and exercise habits haven’t changed dramatically.

The issue isn’t simply the number on the scale. Women may notice:

Changes in where fat is stored, particularly around the abdomen

            Loss of muscle mass

Greater difficulty maintaining their previous weight

Aging itself plays an important role in these changes, as do activity level, nutrition, sleep, genetics, and hormonal changes. Menopause isn’t the sole explanation, but it is an important part of the picture.

This is one reason why preserving muscle mass, strength, cardiovascular fitness, and metabolic health becomes increasingly important during midlife.

Why recognizing the full picture matters

One of the challenges of menopause care is that there is no single “menopause symptom.”

A woman may have classic hot flashes and night sweats. Another may primarily notice insomnia, anxiety, brain fog, and joint stiffness. Another may have significant vaginal and urinary symptoms but no vasomotor symptoms at all.

And sometimes several seemingly unrelated symptoms appear around the same time.

That doesn’t mean every symptom occurring in midlife is caused by menopause. Thyroid disease, anemia, sleep apnea, depression, medication effects, nutritional deficiencies, autoimmune disease, and other medical conditions can produce symptoms that overlap with menopause.

Good menopause care recognizes patterns, considers the alternatives, and treats the symptoms that are actually affecting a woman’s quality of life, rather than attributing everything to hormones.

You don’t have to simply “live with it”

Menopause is a normal biological transition, not a disease. But “normal” does not mean women should simply accept symptoms that interfere with their sleep, relationships, work, exercise, or enjoyment of life.

There are effective approaches to many menopausal symptoms, including lifestyle strategies, nonhormonal therapies, and

           hormone therapy

when appropriate. The right approach depends on a woman’s symptoms, medical history, risk factors, preferences, and goals.

At Rivanna Concierge Physicians, we believe menopause care should be individualized rather than one-size-fits-all. We take the time to understand the full picture, not just whether you are having hot flashes.

Because feeling well in midlife isn’t about simply getting through menopause. It’s about helping you feel like yourself and remain healthy, active, and engaged for the decades ahead.

If any of this sounds familiar, we’d welcome a conversation. We engage seriously with the things traditional primary care doesn’t have time for: hormone health, sleep, energy, metabolic changes.

A Seasonal Vaccine Guide

Rivanna Concierge Physicians outlines 2026-2027 flu, COVID-19 and RSV vaccine guidance from the CDC, AAFP and IDSA, including recommendations by age.

Major U.S. medical organizations—The American Academy of Family Physicians (AAFP), CDC, and the Infectious Diseases Society of America (IDSA)—have updated their seasonal respiratory immunization recommendations for the 2026–2027 season.

We’ll outline some information below, but if you have questions about your specific circumstances, we ask our patients to call the Rivanna Concierge Physicians office. For your convenience, all vaccines can be safely co-administered during a single pharmacy visit.

           Influenza Vaccines

For seasonal influenza, annual vaccination is universally recommended for everyone aged 6 months and older. We recommend receiving this in the middle of October, unless you plan overseas travel. In that case, get vaccinated 2 weeks prior to departure.

For adults aged 65 and older, we recommend you receive one of the more potent options for your age group: Flublok, Fluad, or Fluzone. Alternatively, you can take the newly approved mRNA-based flu vaccine called mFluvisa, approved for those over age 50. Though it wasn’t tested head-to-head with the “over 65” options, it performed better than the standard dose flu vaccines. It was associated with more side effects like injection site pain, tiredness, headache, and muscle aches.

Nasally-administered FluMist is available for those aged 2 to 49 who do not have certain medical problems and are not pregnant. Pregnant individuals should receive an inactivated or recombinant flu vaccine during any trimester.

           COVID Vaccines

For those continuing vaccination against SARS-CoV-2 (COVID), there should be updated vaccines available for Fall administration shortly. Vaccination continues to lower the risk of severe illness from the virus. The Moderna and Pfizer vaccines are available along with a non-mRNA option from Novavax.

           RSV Vaccines

RSV is a common winter respiratory virus that can lead to more severe illness with advancing age. Vaccines were first released in 2023 and immunization is recommended for all adults aged 75 and older. This vaccine does carry an increased (though still very small) risk of Guillain-Barré syndrome, which is why administration is delayed unless individuals are at an elevated risk of severe disease due to underlying chronic conditions. We favor the Arexvy version from GlaxoSmithKline.

Many patients tell us they miss the kind of medical care that once felt personal—a relationship with a physician who knew them well, had time to listen, and was available when questions arose. Concierge medicine was created to bring that experience back.

At Rivanna Concierge Physicians, concierge medicine is about more than convenience. It is about restoring the physician-patient relationship and providing the thoughtful, personalized care that has become increasingly difficult to find in today's healthcare system.

           How Does Concierge Medicine Work?

Patients pay an annual membership fee that allows their physician to care for a much smaller number of patients than in a traditional practice.

Because of that, we can offer:

            Longer, unhurried appointments

Same-day or next-day visits for urgent concerns

            Direct communication with your physician

Comprehensive preventive care and wellness planning

            Careful management of chronic conditions

Coordination with specialists and other healthcare providers

Most importantly, concierge medicine gives your physician more time to focus on you.

Why Are More Patients Choosing Concierge Medicine?

Today's primary care physicians often care for thousands of patients. The result can be shorter visits, longer waits, and less direct access to the doctor who knows you best.

Concierge medicine takes a different approach. By limiting the size of the practice, physicians can spend more time with each patient, develop stronger relationships, and focus on prevention as well as treatment.

For many patients, the greatest benefit is the confidence that comes from knowing their physician is accessible, familiar with their history, and invested in their long-term health.

           Why We Chose This Model

After more than 30 years each in family medicine, we found ourselves increasingly constrained by a healthcare system that often values volume over relationships.

We founded Rivanna Concierge Physicians because we wanted to return to the kind of medicine that first inspired us to become physicians—

care that is personal, thoughtful, and centered on the individual patient

. By caring for fewer patients, we can spend more time listening, answering questions, coordinating care, and helping our patients navigate an increasingly complex healthcare system.

           Healthcare as It Should Be

At its heart, concierge medicine is about creating the time and space for a stronger physician-patient relationship.

When physicians have the opportunity to truly know their patients, healthcare becomes more personal, more proactive, and often more effective.

That is the vision behind Rivanna Concierge Physicians. We believe patients deserve a physician who knows them, listens carefully, and is available when they need guidance most. We would be honored to partner with you on your healthcare journey.

What Is VO2 Max Testing and Why Is It Important?

Learn what VO₂ max measures, why it matters for cardiovascular fitness and healthy aging, how testing works, and how you may be able to improve it.

VO₂ Max: One of the Most Important Numbers for Healthy Aging

Most people know their blood pressure, cholesterol, or weight. Far fewer know their VO₂ max. Yet this single number can provide remarkable insight into your current fitness, your future health, and even your ability to remain active and independent as you age.

           What Is VO₂ Max?

VO₂ max (pronounced "V-O-two max") is the maximum amount of oxygen your body can use during intense exercise. It reflects how well your heart, lungs, blood vessels, and muscles work together to deliver and use oxygen.

Simply put, it is one of the best measures of your cardiovascular fitness.

The higher your VO₂ max, the more efficiently your body can perform physical activity. While endurance athletes often have exceptionally high VO₂ max values, improving this number benefits everyone—not just runners and cyclists.

           Why Does VO₂ Max Matter?

Research has consistently shown that VO₂ max is one of the strongest predictors of long-term health and longevity.

People with higher levels of cardiorespiratory fitness generally have a lower risk of heart disease, stroke, type 2 diabetes, and premature death. In fact, low cardiorespiratory fitness has been shown to be as significant a health risk as many traditional risk factors, including smoking, high blood pressure, and diabetes.

Many experts in longevity and preventive medicine consider VO₂ max one of the most important measures of long-term health, because it reflects not only how long we may live, but how well we are likely to function as we age.

           VO₂ Max and Healthy Aging

As we age, VO₂ max naturally declines. If that decline becomes significant, everyday activities can become increasingly difficult.

Walking up stairs, carrying groceries, traveling, hiking with family, or simply keeping up with grandchildren all require a certain level of aerobic fitness.

People who maintain a higher VO₂ max throughout life are more likely to remain active, independent, and physically capable well into older age. Rather than simply adding years to life, improving VO₂ max helps add life to those years.

           How Is VO₂ Max Measured?

The most accurate way to measure VO₂ max is through a graded exercise test while wearing a specialized mask that measures the oxygen you inhale and the carbon dioxide you exhale.

During the test, you walk on a treadmill or pedal a stationary bicycle while the intensity gradually increases. Your heart rate, breathing, and oxygen consumption are measured throughout the test.

The procedure is safe, non-invasive, and typically takes less than 20 minutes.

Can You Improve Your VO₂ Max?

           Absolutely!

Unlike some health markers that are largely determined by genetics, VO₂ max can often be improved with regular exercise.

           The most effective strategies include:

Regular aerobic exercise such as brisk walking, cycling, swimming, or jogging

Interval training that includes short periods of higher-intensity effort

           Why We Measure VO₂ Max

At Rivanna Concierge Physicians, we believe that preventing disease is only part of the picture. Our goal is to help patients maintain strength, mobility, and independence throughout life.

VO₂ max testing gives us an objective way to measure one of the most important indicators of healthspan—the years you remain healthy, active, and capable. It helps establish a baseline, identify opportunities for improvement, and track progress over time.

Whether your goal is to lower your risk of disease, improve athletic performance, or simply continue doing the activities you love for decades to come, understanding your VO₂ max is an important step.

           Looking Beyond the Numbers

VO₂ max is not about training for a marathon or competing with others. It's about understanding how well your body functions today and taking steps to improve how you'll function tomorrow.

Healthy aging isn't just about living longer. It's about living better—maintaining the strength, endurance, and independence to enjoy the people and activities that matter most. VO₂ max testing provides a valuable window into your overall fitness today and helps guide a plan to improve your healthspan for years to come.

Menopause and Hormone Therapy: What Every Woman Should Know

Menopause is a natural stage of life, but that doesn't mean you have to suffer through it.

As estrogen levels decline, many women experience symptoms that affect their daily lives.

Hot flashes, poor sleep, brain fog, joint aches, mood changes, vaginal dryness, painful intercourse, and decreased libido are all common

. Some women have only mild symptoms. Others find menopause significantly impacts how they feel each day.

The good news is that effective treatments are available.

           Is Hormone Therapy Safe?

This is the question I hear most often.

When it comes to vaginal estrogen, the answer is absolutely yes, for every patient.

For systemic therapy (patch, pill, topical cream/gel), the answer requires more nuance. For the majority of women, the answer will be yes, but it’s important to evaluate and counsel each woman individually.

Much of the concern about hormone therapy came from the Women's Health Initiative study published in 2002. Early reports raised fears about breast cancer and heart disease. Looking back, 20 years later, this study wasn’t interpreted in the best way, from a public health perspective. Today, major medical organizations agree that hormone therapy is safe and effective for many women when prescribed appropriately. The key is choosing the right treatment for the right patient.

           What Are the Benefits?

Hormone therapy is the most effective treatment for hot flashes and night sweats. It can also:

For some women, testosterone therapy may also be appropriate. Although it is not routinely recommended for every menopausal woman, carefully prescribed testosterone can improve sexual desire in women with persistent low libido after other causes have been addressed. It should be prescribed thoughtfully and monitored regularly.

Hormone therapy may not be recommended for some women, including those with certain types of breast cancer, unexplained vaginal bleeding, active liver disease, or a history of blood clots or certain strokes.

At Rivanna Concierge Physicians, we take the time to understand your symptoms, discuss the latest evidence, and develop a treatment plan that fits your goals and your health history. For some women, that includes hormone therapy. For others, it may not.

Explore the cost and value of concierge medicine, including longer visits, direct physician access, proactive care, coordination, and peace of mind.

           Is Concierge Medicine Worth It?

For many patients, the idea of paying an annual membership fee for primary care raises an understandable question:

“Why would I pay for a doctor when I already have insurance?”

It’s a fair question—and one worth answering honestly.

We believe the real value of concierge medicine is not simply about access. It's about restoring the kind of thoughtful, relationship-based medical care that has become increasingly difficult to find in today’s healthcare system.

           Key Takeaways

✓ Concierge medicine is about more than access—it's about time, relationships, and personalized care.

✓ We maintain a smaller patient panel so we can spend more time with each patient.

✓ Membership supports proactive care, prevention, and care coordination.

✓ Many patients view the fee as an investment in their health, independence, and peace of mind.

           The Traditional Model Is Strained

Most primary care physicians today are working within a system that rewards volume rather than relationships.

To keep practices financially viable, many physicians are required to see 20-30 patients per day—sometimes more. Because of these short visits, administrative burdens increase. Phone calls go unanswered for longer. There's no time for deep conversations about your healthcare goals and preferences.

Physicians feel it. Patients feel it.

After decades practicing in the traditional model, we saw firsthand how difficult it became to provide the level of care our patients deserved.

Concierge medicine allows us to practice differently.

What Patients Are Really Paying For

The membership fee is not simply paying for office visits. In fact, we still accept insurance for covered medical services.

The membership supports something much more valuable:

significantly more time with your physician

            direct physician access

            prompt appointments

            proactive care and prevention

            care coordination

            continuity and relationship-building

            thoughtful decision-making without feeling rushed

It allows us to keep a smaller patient panel so we can truly know our patients and respond when they need us.

           Looking Beyond the Fee

           Our membership fee is:

$4,000 annually for an individual adult

$7,500 annually for an adult couple

For some families, that may initially feel like a luxury. But many patients ultimately view it differently—as an investment in their health, time, and peace of mind.

           For approximately $11/day.

Patients tell us the value comes from avoiding:

unnecessary urgent care or emergency room visits

            delays in diagnosis

            fragmented specialist care

            repeated testing

            missed preventive opportunities

hours spent navigating the healthcare system alone

There is also a less measurable, but equally important, value in maintaining health, function, and independence as we age. Helping patients achieve their long-term goals requires understanding what they want their future to look like, evaluating whether they're on a path to get there, and working together to make adjustments when needed. Meaningful preventive care and long-term planning take time—something that's increasingly difficult to provide in the traditional healthcare model.

A strong relationship with a trusted primary care physician can help patients stay healthier and address concerns earlier—often before they become larger and more costly problems.

The Non-Monetary Value May Matter Even More

           Healthcare is deeply personal.

Many patients are not simply looking for prescriptions or lab work. They're looking for:

a physician who knows their history

            someone who listens carefully

            guidance through complex decisions

            help coordinating specialist care

            reassurance during difficult moments

continuity over years, not isolated visits

In a concierge model, patients are not competing with an overwhelming volume of appointments and administrative demands. That changes the experience of care dramatically.

The relationship itself becomes the foundation of better medicine.

Concierge Medicine Is Not For Everyone—And That’s Okay

Concierge care is a personal decision and not the right fit for every patient. But for patients who value accessibility, continuity, prevention, and a close partnership with their physician, the experience of care can be transformative.

At Rivanna Concierge Physicians, that is exactly what we set out to build—the kind of comprehensive, attentive, relationship-centered care that first inspired us to become physicians more than 30 years ago.
